How to Send Your Gift

Make Your Check Payable

Write your check payable to Boots to Health Foundation. We are a registered 501(c)(3) nonprofit — Tax ID 92-1265877.

Use the memo line to dedicate your gift — for example, “In honor of [name]” or “In memory of [name].” We’ll record and acknowledge the dedication.

Mail It to Us

Send your check to our mailing address below. We collect mail every two weeks — include your return address or email so we can send your receipt promptly.

Receive Your Tax Receipt

Because we collect mail every two weeks, please allow up to 3 weeks from mailing for your written acknowledgment to arrive. It’s suitable for your tax records.
